You know, it may very well have been, as a sort of otherwordly reflection of the girl sitting across the bed. It was quite rare to see death personified as a woman, and probably even more rare to see it as a provider of solace, comfort and understanding. My sister could probably analyze this painting nine ways to Sunday (that is her specialty) but I found that after I got over my initial shock, I started liking it more and more every time I saw it... :)
